Find out morePre-admission is an important part of your day hospital care. Your specialist will explain your procedure and complete the consent form with you during your consultation. If you have any questions about your planned day procedure admission, the completion of forms, costs or health insurance status, please contact our team who will be happy to assist you.
To ensure we can confirm your admission, health insurance and other details/arrangements, we ask that you please complete either the online pre-admission form, or paper admission form at least 7 days before your admission.

If you would prefer not to complete the online form, you can send us the paper forms usually provided to you by your surgeon. If you don’t have the paper forms please contact us before your admission date
Prior to admission be sure to confirm your level of cover for hospital admission with your health insurance provider. Here are some questions to ask:
At least 5 days before admission:
Hobart Specialist Day Hospital will SMS or phone you (if you have not advised a mobile) confirming: time of admission, fasting information and any hospital costs to be paid on admission. Note that your surgeon and anaesthetist will invoice their fees directly to you.
We accept Visa and Mastercard but do not accept AMEX or personal cheques. When we contact you we will advise you of the hospital costs for your procedure, known as your Informed Financial Consent.
These costs cover the use of the operating theatre, day-stay accommodation and commonly used drugs and dressings. For some procedures, there will also be a charge for prostheses or disposables.
If you have private health insurance we will claim directly from your health fund on your behalf and you do not have to do anything.
If you have an excess or gap with your level of cover you will need to pay this on admission. Your health fund will have advised us directly of the cover they will pay.
If you do not have private health insurance you will need to pay your costs on admission.
Please be aware that your specialist, anaesthetist and/or pathologist will send their accounts for their services separately and directly to you. The hospital does not handle these accounts and you will need to pay them directly.
Please note that when you receive these doctors’ accounts they will list the same item numbers as on your hospital account, but they are for your doctor’s services, not the hospital’s costs.
